Before completing this form to postpone a game please ensure you have notified your opponents and the referee (if appointed) that you are requesting the postponement of a game that involves them.

All clubs requesting a postponement or failing to fulfil a fixture must complete the Postponed / Unfulfilled Match form (below) for the relevant age group from the list above.
